Introduction
The Georgia Department of
Community Health (DCH) is pleased to announce a program to deploy an Electronic
Health Record Solution (EHR) integrated with a statewide Health Information
Exchange (HIE) to be offered to physician practices serving our state's
Medicaid beneficiaries. We plan to
kick-off our program with the initiation of Phase I in which approximately
thirty qualified providers will be selected to participate in the
implementation of the EHR solution within their practices.
Please complete this form with
information requested on your practice.
Once you have gathered the information requested, it should take no more
than 30 minutes to complete. This information
will be used by GA DCH to determine your practice’s specific requirements and
readiness for participation in the Georgia Healthe Connect Phase I. Understanding your current practice profile
and use of information technology (IT) is necessary to evaluate how best to move
forward with creation of what will ultimately become a seamless network for
information flow between the various hospitals, clinics, physicians’ offices,
and other facilities throughout the state.
As you complete this form, please note that “Providers” refers to Physicians, Nurse
Practitioners, Physician Assistants, Audiologists, Optometrists, Physical
Therapists, Music Therapist, Speech Therapists, Psychologists, Dentists,
Hygienists, Licensed Social Workers, Midwife, Nutritionists, Dietitians,
Counselors, Mental Health Practitioners, Neurophysiologists, and Podiatrists
employed by or under contract with the practice. This does not include
personnel employed by or under contract with the practice as office managers,
secretaries, or
other administrative staff, or
Nurses (other than Nurse Practitioners).
Please also note that “specialties” refers to medical
specialties, e.g.: Primary Care, Internal Medicine, Pediatrics, OB/GYN, etc.
